Getting Started With Sunshine Imaging
1. Call Sunshine Imaging to schedule your CBCT scan appointment: (561)-745-7350
2. Complete referral form and fax at least 1 day prior to the appointment: (561)-743-8399
- A referral form can be found in your Sunshine Imaging Folder
- Go to www.sunshineimaging.com
- Click the “download the referral slip” link on the right hand side of the screen
- We will make sure you have a supply of our referral forms for your office.
3. Patient is scanned.
4. After the scan your practice receives the scan information (full volume) on a viewer disc within 24-48 hours, which allows any image to be created for treatment planning anytime. *If requested we will convert CBCT data to DICOM format. These DICOM files can be converted and integrated with 3rd party viewing or treatment planning software including, but not limited to:
- iDent
- Anatomage
- Nobel Guide
- Easy Guide
- Simplant
5. Payment Policy: Payment for the scan and other diagnostic services is required at the time of service. We require that your office credit card be kept on file for this purpose. Payment by check will also be accepted.
6. Cancellation Policy: Sunshine Imaging is sensitive to changes in the doctor’s schedule beyond his or her control; however, we request that all cancellations be made at least 24 hours in advance so we can use the allotted time for another patient’s needs.
